In “The Essence of Spiritual Life,” Swami Rama explains that to do selfless service one needs to cultivate two qualities: non-attachment and a loving attitude to do selfless service. Selfless service requires that an individual perform any service without any attachment or expectation and remain unaffected by the results of such service. At the same time, such an individual needs to cultivate a loving attitude toward such a selfless task, without developing any stress from it, no matter the outcome. However, if you do not love doing some task and yet you do it, it creates a conflict in mind leading to an emotional upheaval. So learn to create love toward your selfless duties. Furthermore, notice if you are happier and satisfied at the end of the day because this is what true karma yogis have experienced; seek them out and you will commonly hear them saying that the more they serve selflessly, the more true happiness they receive. Dalai Lama and Mother Teresa come to mind if I am asked to name some karma yogis.

The essence of the Bhagavad Gita, which is repeated throughout the text, emphasizes the benefits of selfless service. Here is how Krishna puts it, regarding selfless service in Chapter 2:

“Set thy heart upon thy work, but never on its reward, Work not for a reward; but never cease to do thy work. Do thy work in the peace of Yoga and free from selfish desires, be not moved in success or failure”—translated by Juan Mascaro.


And this approach—performing a task without any expectation, letting go of all results, whether good or bad, and focusing on the selfless action alone—is the essence of karma yoga. In the light of non-attachment, the selfless doer attains freedom from emotional disturbances, including but not limited to desires, ambitions, fear, worry, anxiety, judgment, rage, etc. This leads to true happiness.

A recent research study on volunteerism and its effects on longevity, "Motives for Volunteering Are Associated With Mortality Risk in Older Adults,” by Konrath, et al,  confirms the importance of selfless service as advocated by the Bhagavad Gita. According to this study, people who volunteer may live longer than those who don’t, as long as their reasons for volunteering are to help others rather than themselves. Researchers analyzed data from the Wisconsin Longitudinal Study that followed a random sample of 10,317 Wisconsin high school students from their graduation in 1957 until the present. In 2004, respondents reported whether they had volunteered within the past ten years and how regularly. Respondents also mentioned their reasons for volunteering by answering ten questions. In that list were some questions regarding motives that were more oriented toward others and some that were more self oriented. The researchers then determined how many of the respondents were still alive in 2008. Following were the observations drawn from this interesting study:
  1. Nearly 2.3 % of the volunteers had died, compared to 4.3% of non-volunteers. The frequency of volunteer time mattered as well; less than 2% of the regular volunteers had died as opposed to 2.5% of occasional volunteers.
  2. The participants who volunteered only for compassionate reasons achieved the most health benefits. However, the ones who volunteered purely for personal gain or self growth were as likely to die as those who didn't volunteer at all.
  3. Respondents who listed social connection or altruistic values as their predominant motive were more likely to be alive compared with non-volunteers.
According to one of the authors of the study, “It is reasonable for people to volunteer in part because of benefits to the self, however, our research implies that, ironically, should these benefits to the self become the main motive for volunteering they may not see those benefits.”

It is easy to understand why volunteering with an altruistic attitude helps an individual to live longer:
  • Stress reduction. When helping others, the body releases an important hormone called oxytocin, which assists in buffering stressful thoughts.
  • Morale. Merely thinking of a selfless service releases certain “feel-good”chemicals namely dopamine and serotonin (lack of which have both been linked with depression and other mood disorders.
  • Self confidence. Self-esteem builds and confidence levels grow when you are passionate about something and helping others in need
  • Health and pain reduction. Helping others has proven to help with chronic pain and cardiovascular health.

Because our ability to balance changes as we age, and poor balance is at the top of the list of risk factors for an increased chance of falling, it is obvious that maintaining and improving balance is a worthy endeavor! For almost everyone I talk to about their goals as they age, staying independent in function and lifestyle is at the top of their list. Sadly, at the extreme, poor balance often leads to unexpected falls, which can result in devastating fractures and loss of both physical function and being able to live independently. But deteriorating balance can just as easily erode the ability to go about your daily activities with ease and to engage in healthy physical activities. I am sure many of you have friends or family members whose changing ability to balance gradually kept them home bound and had a negative impact on their quality of life. That's why we consider balance as one of the four essential skills of Yoga for Healthy Aging.

Aging Changes to Our Balance System

Before getting into the ways yoga can maintain and improve balance, it is worth reviewing how balance changes as we age, in an honest light. In our post Aging and Balance, Shari Ser, who both a physical therapist and a yoga teacher, said:

“Unfortunately decline in ability to balance is part of ‘normal aging.’ Physiological changes inevitably occur in aging; the composition of the muscle fibers changes from fast twitch to slow twitch, which means our reaction times decline. There are changes in peripheral nerve conduction speeds, and we may either overcompensate or under compensate when our balance is challenged. The smooth modulation of our central nervous system is affected as we age. There are intricate feedback systems between our cerebellum, brain stem, cranial nerves, and basal ganglia that need to be working smoothly for us to easily execute our locomotion and peripheral body through space. 

Our proprioception (the ability to sense where we are in space) also changes as we age as well as our vestibular system (the balance system that is our inner ear). Some hypothesize that this is due to circulatory changes as we age. There are changes in our auditory and visual systems as we age. We get stiffer in our joints and muscles so we loose the full range of motion these joints may have had previously. Cognitive changes occur in our brain also slowing down the in streaming of information. Balance is a reflexive activity, an unconscious response to a change in stimuli, but to improve balance also requires mental concentration.”

How Yoga Helps

In that same post, Shari suggested some ideas on how yoga helps maintain or improve our balance: 

“The maxim “use it or you loose it” is so true with balance! But if we stand in Tadasana (Mountain Pose) we are integrating every single one of our balance systems. Our eyes orient us to midline and to the horizon, and they tell us where up and down are. Our vestibular system tells us that the surface is stable and there is no spinning or tilting to compensate for. Our proprioceptors in our joints, muscles, and skin send feedback to the resistance of the floor, and our conscious mind sends information to elongate here, relax there, shift to the right/left, etc. Our brain is never quiet in Tadasana, always tweaking a little bit here and there. The act of standing still is a full body isometric activity, and it is work that is making our bodies stronger.” 

We can challenge our bodies even more when we stand on one foot, balance on one foot and a hand, such as with Half Moon pose (Arda Chandrasana), or move through a series of connected postures, as in Sun Salutations, because balance and agility are intimately interconnected. In fact, agility requires balance in motion. As Leslie Kaminoff observes, as we move through our day, it is as if we are almost falling and self-correcting moment by moment, and our balance system is what keeps us upright. 

By engaging in stimulating all aspects of our balance system to work at optimal levels, a regular yoga practice has the potential to influence all of the aging changes Shari outlined above. With yoga, you can:
  • Stimulate the pressure receptors and proprioceptors in your feet by working barefoot in balance poses barefoot.
  • Stimulate the mechanoreceptors in our joints, ligaments, tendons, and muscles above the feet via the various positions in which you place your limbs during yoga asana.
  • Improve mental focus by training your minds to return again and again to the balancing task at hand. You notice shifting sensations, the feeling of being on or off balance, when extraneous thoughts take you away from your focus on balance, and so on. Even the pranayama and meditation practices you do can lead to improved mental focus when you find yourself doing balancing poses or walking around the house. 
And yoga can address the sports medicine concept of “controlled instability” News on Balance, Falling, and Yoga) by initially introducing someone new to yoga and previously inactive to the basic yoga poses and dynamic sequences, then adding more challenging poses and practices for the advancing practitioner, and finally, when balance poses are mastered, enabling the practitioner to challenge the edge of stability by creatively modifying poses.

By simply getting you out of your shoes and socks, yoga helps you wake up the balance and pressure receptors in your feet in general, whether you are ready or not for full balance poses. So even practicing standing poses in general is helpful for beginners. And for more advanced practitioners, yoga helps wake up the balance and pressure receptors in your hands when you practice arm balance poses, such as Side Plank pose (Vasithasana), Crane pose (Bakasana), or Handstand. 

Other factors that affect balance and safety are our individual levels of flexibility, strength, and endurance. And since we know that yoga can improve these factors, their improvement will contribute to improved balance as well.

Our mental attitude towards balance and the risk of falling can also have a huge impact on our success or failure, our progress or lack thereof. The yoga practices of mindfulness without judgment that we try to cultivate can change a habitual negativity around balance, such as “I suck at Tree pose,” to a more neutral mental input, like “I am typically wobbly in Tree, but let’s see what I can do today….”

When Grant Place Press contacted me about reviewing Oxford Messed Up by Andrea Kayne Kaufman, little did I realize the connections I would find with my own life.  The protagonist, Gloria Zimmerman, is traveling to Oxford to study feminist poets, and struggles with OCD.

I have a degree focused on feminist poets.  Reading poems written by these women, and writing my own poems gave me strength to deal with depression, OCD and other pain.   I was a very serious young woman, who didn't know she had OCD, but knew that I was often scared about symptoms in my body, and trying to be perfect.  Gloria knows she has OCD, but attempts to keep herself together with rituals, Klonopin, and Van Morrison's music.

Van Morrison's music is a source of strength to both Gloria, and her "loo-mate" Henry Young(the other protagonist who has had many traumas in his life).   As a girl, I  remember sitting on the floor going through my parents record collection, trying to understand what a Moondance was, and mesmerized by the album.

I was intrigued that one of the recommendations on the back of the book came from Susan Richman from OCD Chicago, a chapter of the International OCD Foundation.  Oxford Messed Up does indeed explore Exposure and Response Prevention Therapy(ERP), the standard for treating OCD, and is the only novel for the adult market that I've ever come across that does.

Henry is the catalyst for Gloria's ERP, and this shows the importance of rapport for willingness to do Exposures.  If I hadn't trusted my ERP therapist, I wouldn't have done any of the Exposures he suggested.  I believe that family and friends are often on the frontlines of working on Exposures with the ones they love who have OCD, and that self-help has been very important for many sufferers, especially Jonathan Grayson's book Freedom from OCD, and his model of GOAL support groups.

At the same time, though, I was ambivalent about Henry's quick study of ERP, and how unrealistic this might be.  I was able to suspend disbelief because I remembered reading somewhere that ERP, even incompletely done, can be better than years and years of talk therapy. I only wish more therapists were  quick as Henry--like the 6 or 8 of them I saw before I found Leonard, who actually specializes in OCD and was expert in Exposure Therapy, rather than asking me to simply avoid my triggers or use logic to make it all go away.  I would imagine a novelist's dilemma would be how to portray a series of ERP sessions with a therapist, in a way that would make good fiction.  

Gloria has contamination OCD, which I don't have, and which gave me a brief flash of disappointment, because this is the stereotype of people who have Obsessive Compulsive Disorder.   But she also has perfectionism, a mandate to be a "Superstar,"  which I identify with, and I liked that Kaufman showed how perfectionism can underlay everything, and that this may be slower to respond than other aspects of OCD.  If patterns have been in place for a lifetime, through family culture, personal history, combined with genetic heritage, that simply habituating to the "dirty world" isn't enough, and that compassion, love, understanding and hope are crucial to regaining a life that has been ordered by taking as few risks as possible, and trying to earn the love of others.  

I recommend Oxford Messed Up for it's improvisational dialogue both internal and external, for an inside look at the OCD thought process while facing fears, for the importance of hope, and of "fatalistic optimism"(as the characters describe the music of Van Morrison).

He sent me a copy of a report from Medscape Medical News that was published online following a meeting of the American Academy of Ophthalmology and Middle East Africa Council of Ophthalmology 2010 Joint Annual Meeting, held during last year’s AAO Meeting.

Intravitreal Injections Expose Patients to Streptococcus More Often Than Eye Surgery

Streptococcus is isolated much more frequently from endophthalmitis cultures after intravitreal injection (IVI) of antivascular endothelial growth-factor agents than after ocular surgery, results of a meta-analysis show.

Ophthalmologists should consider using additional sterile techniques during these injections to prevent exposing patients to the sight-threatening complication of streptococcal endophthalmitis, said study author Colin McCannel, MD, associate professor of ophthalmology from the Jules Stein Eye Institute at the University of California at Los Angeles.

"If we can prevent some of the worst cases of endophthalmitis, I think it's worth the effort," Dr. McCannel told Medscape Medical News.

To confirm his impression that reports of Streptococcus organisms seemed more frequent than should be expected after IVI, Dr. McCannel analyzed the American medical literature from 2005 through 2009. He found 16 articles that reported the causative organism in post-IVI endophthalmitis. As expected, endophthalmitis was rare, occurring in 54 of 105,531 injections. Only 26 were culture-positive, according to the abstract. Most of the causative organisms were coagulase-negative Staphylococcus, he said.

However, cultures yielded Streptococcus organisms 30.8% of the time (8 of 26), which Dr. McCannel said is "3- or 4-fold higher" than the incidence reported in the literature for acute postoperative endophthalmitis. The postoperative incidence of streptococcal endophthalmitis ranges from 0% after vitrectomy to 8.2% to 9% after cataract surgery, he reported.

Likely Source of Infection

This finding led him, he said during an interview, to do "some detective work" to try to find the possible source of these streptococcal infections. He found several studies in the anesthesia literature reporting streptococcal meningitis after dural puncture, a procedure that, according to Dr. McCannel, has a working distance between physician and patient similar to that of IVI. Analysis found that the causative organisms in most of those cases came from the treating physician's oral flora, which was aerosolized during talking.

Consequently, the Centers for Disease Control and Prevention in Atlanta, Georgia, recommended in 2007 that spinal procedure operators wear a surgical mask during the procedure (MMWR Morb Mortal Wkly Rep. 2010;59:65-69).

Ophthalmologists "often have to give directions to the patient during injections, and sometimes there is small talk," Dr. McCannel said. "The patient's eye is probably being showered with these microscopic droplets. We may be contaminating the injection field or the needle."

Recommendations

If other studies validate his findings, the ophthalmic community should decide whether to recommend wearing a surgical mask during IVI, he said. He told the audience that he does not wear a mask because "it would be burdensome to take it on and off" during the brief injections.

Another precautionary strategy that Dr. McCannel suggested was to avoid talking, coughing, or sneezing during the injections.
