Built on Sinking Sand The House of OCD and Health Anxiety

Sunk and askew

It was the summer of 2006 when I sank deeply into the OCD with a full scale health anxiety crisis. OCD thinking is like an elaborate structure built on sinking sand. It appears so real and solid and indisputable and all the while your life is collapsing underneath you, and the logic is built on inherently unstable ground. OCD is an entire industry of thinking with very little to do with the outside world, except for the grain of truth that it inevitably latches onto.

Even writing about this summer stirs up the OCD. Part of my compulsion to make the anxiety go away was to make detailed notes about my symptoms, in hopes that I accounted for every possible thing, so my impulse, when writing this blog post, is to include every detail. The anxiety began when I agreed to take a medication which might alleviate a symptom that I obsessed about frequently. I took the medication, and started obsessing that I was getting a bladder infection, which was another one of my health anxiety themes. After fixating on every bladder sensation, and agonizing about whether to call my doctor, I finally did and she got me in at the end of the day. It is common(though perhaps not recommended) to treat women empirically if they have bladder infection symptoms, so that's what she did--no urine test.

And that began an elaborate building of health fear, one level on top of another. Because I'd had a severe headache with one antibiotic in the past, I said I wanted a different one, and in dodging one fear I landed in another as I began to obsess about getting a yeast infection from the 2nd antibiotic, and called my doctor who prescribed meds over the phone. I was overwhelmed with a surge of anger at my doctor for not doing a urine test since I had several bladder infections that turned out not to be, and since strong feelings set off my OCD, I was on very shaky ground.

Over the next weeks the OCD was compounded and grew at an alarming rate. I went to a different gynecologist, who was near my house, who did a lab test and said I did not have a bladder infection, and gave me more meds for the supposed yeast infection. I still felt urgency in my bladder, and even more urgency in my OCD desire to know exactly what was going on in my body, and perfectionistic self-condemnation for not perfectly diagnosing myself or advocating for tests, or for calling a doctor in the first place. The OCD thoughts were all about my defectiveness, and my responsibility to make sure I didn't get a kidney infection, and the imperative that I never make a mistake, and that I would be haunted by the pain and discomfort in my body.

I was on a quest to get relief from my anxious imaginings and my fixation on painful sensations in my body. I found a new family doctor, who said if I still had bladder urgency that I probably did have an infection, and she wrote a script for antibiotics, and reluctantly did a urine test at my request, but when I panicked that I did not do the test sample correctly, she went into authoritarian mode and told me to not get myself worked up, to just calm down. Well, if I could do that, I think I would have already. . .I went and cried in my car.

So here I was trying to decide whether to take another antibiotic, without test results, and the OCD structure was growing, as I felt a surge of hyperresponsibility that if I got a kidney infection it would be all my fault, so I'd better take the antibiotic. By the end of that week, I couldn't sleep at night, and I went into the doctor on a Saturday. The doctor on call was much calmer and kinder, and she prescribed another antibiotic that would be less likely to cause insomnia, but this antibiotic wreaked havoc with my digestive system. I went back to the doctor on call who happened to have my test results, and once again it showed I didn't have a bladder infection but that I did have blood in my urine, and she wanted me to see a urologist.

By this time I was a mess. My therapist Molly was on vacation. I went to see the therapist on call, and told her how I couldn't get this right, couldn't figure out what was wrong with my body, that I needed to try harder. She sat quietly for a moment, and then said that it seemed I had been working incredibly hard to find out what was wrong, with multiple doctor visits and relentless researching on the computer. I was baffled by this. This did not make sense in my OCD world, where I was never good enough, never finished, never solved, never certain.

She wrote a few sentences on a scrap of paper, and gave it to me:

May I be filled with lovingkindness.

May I be safe from inner and outer dangers.

May I be well in body and mind.

May I be at ease and happy.

This was a glimmer from outside the windows of the house of OCD, the possibility of another way to view the world. Feeling body symptoms can be hard enough without the "inner dangers" of OCD. I was to descend further into my OCD, but I had a moment of reprieve.

1 Year of Exposing OCD!

Not that way

I began this blog, Exposing OCD, a year ago. I have learned immensely from the process of writing the posts, reading and responding to comments, and in discovering other bloggers who write about the experience of having OCD. I was initially inspired to start a blog when I came across The Beat OCD Blog: Overcoming OCD One Exposure at a Time. The author was blogging anonymously and about doing exposures, and it was like a door opened to the idea that I could write about my experiences as well. I had been writing articles for an OCD newsletter, but it came out only a few times a year, and I'd noticed writing had been helpful for gaining distance from my old patterns. I'd like to give a shout out to The Beat OCD Blogger(or Ann as she now calls herself), and her incredible persistence in doing exposures and reclaiming her life from exhausting OCD rituals and avoidance of the living she really wanted to do.

I wanted to write about my experience doing Exposure and Response Prevention Therapy(ERP), and as a motivator to actually do more exposures. Ironically I started the blog while at work, where my compulsing ate up many of my days, and avoidance was mixed into my other aims. Writing the blog was a distraction, but as readers found my blog, it helped focus my attention on the nature of OCD as a disorder, especially mental obsessions, intrusive thoughts, indecision, perfectionism and health anxiety. OCD is still a mystery to many people, and the most discouraging is how many therapists also find it a mystery, and either don't realize their client has OCD, or doesn't know about ERP. I hope that in some small way I am helping to describe OCD experiences such a way that others can recognize what they may have found impossible to articulate, and to offer hope that it's possible to get better, to be an inhabitant of your own life, rather than living a life of compulsions.

Writing each post was an exposure, since my perfectionism often had resulted in avoiding writing anything unless it was "perfect," along with fears of saying the wrong thing, or choosing the wrong topic or the incorrect photo. I have been honored by people's willingness to share their own experiences with me by leaving comments on the blog, and encouraged by the number of OCD blogs I've found over the course of the past year. My blog roll has grown, and although I wouldn't wish OCD on anyone, I am thankful for companions for the journey of courage in facing OCD.

Editorial Comment The Lasermedics Press Conference – The Rest of the Story

This editorial comment was prepared for publication in the October 1994 issue of Medical Laser Report, but was not published. (The editors of the newsletter chickened out!)

Editorial Comment -- The Lasermedics Press Conference – "The Rest of the Story"

Irving J. Arons
Managing Director
Spectrum Consulting

Last month, our front page feature story was about the release of the results from the General Motors carpel tunnel syndrome (CTS) study. I was the only media person allowed to attend the symposium held by Lasermedics, as GM pulled the plug on all publicity about the event two days before it occurred (on August 31). I was allowed to remain in the room when the study results were presented, only because I had agreed to allow GM to see, comment, and have some editorial control on what I wrote. This is not my usual custom, although I sometimes send drafts to the companies or people about whom I am writing for their comments and corrections in order to accurately report about the subject on which I am commenting.

In this case, GM was extremely concerned about the release of the information from the symposium for several reasons: the "implied" endorsement of the Lasermedic's laser; the effect of the "good" results on their ongoing negotiations with their labor union and the effect this "benefit" might have on those negotiations; and the release of data before their in-house scientists could prepare the information for publication in a scientific journal. For these reasons, the national and local media were excluded from the proceedings. I was able to "negotiate" my presence only on the terms noted above.

The original draft that I prepared was scrutinized carefully by GM personnel at the highest levels within the organization, and after lengthy negotiations, the "sanitized" version, which was published, was reluctantly approved by GM.

However, now that the "cat is out of the bag", I would like to fill you in on "the rest of the story", or what was "altered" or omitted. According to the Bureau of Labor Statistics, as noted in the Wall Street Journal of July 14, 1994, approximately 9% of auto manufacturing workers are afflicted with repetitive stress injuries, including carpel tunnel syndrome. And according to my sources, that percentage holds true for GM assembly workers as well.

As for the double blind clinical study conducted at eight GM plants, 116 out-of-work employees volunteered to take part in the study. All were chronic CTS sufferers, some out of work as long as three years because of their disability. Eighty-six completed the five week program and, of those, 46% of both groups (47% of those that had physical therapy alone [and treatment with a "sham" laser -- having the aiming LED light active but the laser inactive] and 45% of those that had both physical therapy and laser treatment), were able to return to work. However, this number is misleading because many more of those treated would have been able to return to work if appropriate jobs had been available or if seniority allowed. Thus the 46% is just a baseline, as the percentage would have been considerably higher if jobs had been available.

More importantly, I was told by a plant manager that the laser continued to be used in the eight GM plants (the study concluded last fall) and that thousands of workers complaining of CTS symptoms, i.e., in the early stages of CTS development, were being treated, with a success rate of between 85% to 90% of alleviating the symptoms and allowing the workers to return to their jobs!

I believe this information is important and should have been presented, and that is the reason for this editorial.

The Pros and Cons of Manual Adjustments by Yoga Teachers

by Nina
Study of Hands by Albrecht Durer
We recently received a question regarding our policy on manual adjustments:

I was wondering how you (or any of the bloggers) feel about yoga teachers giving hands-on adjustments in a group setting? I have been teaching for 13 years but I do not feel comfortable giving adjustments. It just strikes me as too risky because I can't possibly know what's appropriate for any student's body. I'd much rather rely on detailed verbal cues and a slow-to-moderate pace that allows students to align and adjust their own bodies. Occasionally, I give a light touch if I see that a student is not getting the verbal cues and if I'm concerned that his/her alignment looks dangerous.

Now, just yesterday, I took a class from another teacher in a studio I'd never visited. A friend of mine recommended this teacher. Toward the end of class, without asking my permission, she came over and pushed me deeper into a supine twist. She did this to half the class on one side of the body and the other half of the class on the other side of the body. Afterwards, I just felt a bit lopsided. However, today I noticed that my spine was definitely out of alignment and went to the chiropractor for an adjustment. I still have some residual discomfort. This experience has reinforced my idea that hands-on adjustments in group classes are not very wise. Comments?

To start, I have a very simple answer for this reader, who obviously prefers not to receive or give adjustments. For you I’d say that you should not give adjustments to your students (except the rare light ones you do now). No teacher who isn’t comfortable using manual adjustments should use them. And if you don’t like receiving adjustments or don’t trust teachers to give them to you, then you should inform your teachers that you do not wish to receive them.

But rather than just dismiss the topic with that simple answer, I thought I’d address it in general, because quite frankly I love manual adjustments, both receiving and giving them. I find that they are an extremely effective form of communication, one that can’t quite be replicated with words (and I’m a writer). In my teacher training and apprenticeships, I was trained in the use of manual adjustments, so I’ve always been comfortable using them. And when I was teaching a group of international graduate students and their spouses, I found using my hands to communicate was particularly important because my students typically did not understand subtle, anatomical instructions. I must say, however, that I never give adjustments to “push” anyone deeper into a pose, but rather to communicate a direction of movement or relaxation that I feel will help the student find more freedom in the pose.

As far as receiving adjustments goes, some of my best insights have come from the touch of a teacher’s hand. As I practice at home, I sometimes have a distinct visceral memory of a teacher’s hands guiding my body into opening or relaxing, even years after the original experience. I have had one negative experience when I was trying a new style of yoga and the teacher’s assistant, wanting me to “melt my heart” in a Downward-Facing Dog, pushed quite firmly on my spine (apparently my Iyengar style alignment was not going over well) in a way that made me quite upset. That just put me off that particular teacher—and maybe that style of yoga—but not adjustments in general. Obviously—and we say this all the time on our blog—we need to use discernment in choosing the teachers we study with. Because improper use of adjustments could be potentially injurious, how our teachers use manual adjustments (if they do) should certainly be one of the criteria we use to choose who we study with. So I’d say go with your gut: if you’re uncomfortable in any way with how the teacher uses his or her hands, find another teacher.

But since this is a rather controversial topic, rather than just going on my own experience, I decided to consult some experts. Of course, I just happen to have a few experts on staff, including two who are medical doctors! But everyone on the staff except Brad teaches yoga so I thought it would be interesting and valuable to ask them all. To make it easy, I asked everyone all the same three questions.

DR. BAXTER BELL:

Do you like receiving manual adjustments from your teachers? Sometimes, if they know what they are doing.

When you teach (if you do), do you like to GIVE manual adjustments to your students? Sometimes, if the setting is appropriate: I know the students, they are agreeable.

Optional: Very briefly, do you have anything to say about the pros and cons of manual adjustments? It seems that certain styles of yoga may do a better job of training their teachers in the measured and appropriate use of adjustments. Done well, they can be enlightening for the student; done poorly, injurious.

DR. TIMOTHY MCCALL:

Do you like receiving manual adjustments from your teachers? When it’s from a teacher I trust yes. But there have been times where I have not allowed a teacher to adjust me. I also think there is a crucial difference between gently guiding someone in the right direction, and taking your hands and bring them to the position. I have experienced the latter, and while (when done well) it can feel great, the risk of injury is much higher.

When you teach (if you do), do you like to GIVE manual adjustments to your students? Yes, in the gentler style mentioned above.

Optional: Very briefly, do you have anything to say about the pros and cons of manual adjustments?
Done skillfully they can be great. Done unskillfully it’s a craps shoot.

RAM RAO:

Do you like receiving manual adjustments from your teachers? Yes.

When you teach (if you do), do you like to GIVE manual adjustments to your students? Yes, I always declare before the start of the class that I will be making fine adjustments that require for me to place my hands. If they are not comfortable with me touching, they need to tell me.

Optional: Very briefly, do you have anything to say about the pros and cons of manual adjustments? Manual adjustments are transmitting a message across from the teacher to the student through the hands. How you do it can be interpreted in so many ways:

PROS

A) Helps a student move into the pose
B) Helps a student find his or her balance
C) With manual adjustments a person may achieve new sensations or experience
D) Manual adjustments help the student to go deeper into the pose
E) Helps develop a long-standing teacher-student (guru-sishya) relationship

CONS

A) Genuine intentions of the teacher to adjust can also misinterpreted
B) Weak students or beginners can injure themselves

SHARI SER:

Do you like receiving manual adjustments from your teachers? I like receiving adjustments. It tells me the teacher is looking at me and assisting me in my stuck or over-used areas. It is a key to awareness.

When you teach (if you do), do you like to GIVE manual adjustments to your students? I give adjustments as a teaching tool to encourage the student to direct attention to an area that either is vulnerable or is where they are working too hard.

Optional: Very briefly, do you have anything to say about the pros and cons of manual adjustments?
Adjustments are tricky. You're aren't doing the student's yoga—it has to be their yoga. You are just the guide to keep someone safe. Also, how you learn to touch is very important. Touch has to be non-invasive, minimal and respectful. A lot of people don't want to be touched, and you need to respect that. Judith Lasater always asks the student for their permission to touch, which is extremely important, especially when it is a new student in the class. If these precepts are respected, then there is no con and it is all pro.

BRIDGET FREDERICK:

Do you like receiving manual adjustments from your teachers? I love manual adjustments, but my teachers are superb. I realize that not all teachers have the same finesse. There is a way to gently suggest an energy flow without really manhandling someone.

When you teach (if you do), do you like to GIVE manual adjustments to your students? Yes, because in my own practice I find the adjustments invaluable. Often, with my students, some touch is necessary to get them to find the release or extension I'm seeking. Students can get lost in a pose—connecting words to anatomical movement can be confusing. Often a directional touch can suggest more than a whole paragraph of explanation. But if I'm working with a group of students I do not know, I'll announce at the beginning of class that I tend to be pretty hands-on and that if this is not welcome, to just let me know.

Optional: Very briefly, do you have anything to say about the pros and cons of manual adjustments?
Manual adjustments are energetic and learned over time. Getting adjustments from a skilled teacher is incredible. Bad manual adjustments can be unpleasant, or even cause injury.

BRAD GIBSON:

Do you like receiving manual adjustments from your teachers? Yes, I find this really instructive. It also gives a more personalized feel to a class—especially if it's a large one.

When you teach (if you do), do you like to GIVE manual adjustments to your students? N/A

Optional: Very briefly, do you have anything to say about the pros and cons of manual adjustments? I almost always appreciate the extra instruction of manual adjustments. I suppose the only downside is when this happens from an assistant and I find myself (sometimes) out of sync with the rest of the class.

Causes of Pimples

The causes of pimples is generally similar to the reason people get acne. What causes pimples is usually related to the causes of acne too. Acne is a skin disease that affects the follicles or pores. Follicles cover most of the face and body. A follicle is a tiny skin duct under the skin that a single hair grows out of. Below the duct is a tiny gland that produces sebum, skin oil. This oil is necessary for healthy skin and hair.

What causes pimples is usually the skin below and around the follicles are constantly shedding and growing new skin. These dead skin cells can become excessive and clog up the pores. Contributing to this may be the overproduction of sebum, clogging the pores even more. Once the pores become clogged bacteria form causing a lesion or pimple.

There are several types of lesions. A comedo is a clogged pore. Excess oil mixed with dead skin cells under the surface form a plug. One type of comedo is a whitehead. A whitehead is formed when the clog plugs up the pore entirely forming a small bump with a white center. A blackhead is a clogged pore with a black center. The black center is not dirt but is cause by oxygen having been exposed to the pore because it had opened up slightly.

Papules or pustules are what people usually call pimples. These are raised red bumps because the clogged pores have become infected. The bacteria also cause swelling and bulging. If the pimple is filled with pus it is called a pustule. The pus can become yellow with infection.

Another of the causes of pimples can be over-washing. Over washing acne infected skin can actually produce more acne and more pimples. The skin will make more sebum, which in turn can make acne worse. Wash your face no more than twice a day using acne medications. Never pick or pop a pimple. This will cause the spread of more bacteria by pushing some bacteria deeper into the skin. This will make the pimple last longer and can also cause it to develop into a cyst. Once the pimple is open it is exposed to even more bacteria from the face and hands causing a bigger infection. This is what causes pimples to spread even more.

Sweating can cause acne to worsen. Pores can become blocked from excess sweat that cannot evaporate. Always take a shower after sweating and wear loose fitting clothing. Athletic equipment such as headbands, chin straps, pads and helmets may all trap sweat and contribute to the causes of pimples. Avoid wearing fabrics that don't breathe well such as nylon or spandex. Instead, wear cotton fabrics that help to wick sweat away from the skin.

What causes pimples in women is also their hormones. Hormones can cause the body to produce more sebum than usual. Women may notice that they have an acne outbreak near the time of their period. Hormones often fluctuate greatly during the teen years and often acne will go away once hormones settle down.

Some medications are also what causes pimples, as they can cause acne to flare up. Anti-depressants, steroids, epilepsy medication and some birth control pills can all contribute to acne. Never stop taking medication because of an acne breakout. Instead, talk with your doctor about it so he can adjust your medication or find other possible causes of pimples and acne.

Onigbinde calls for establishment of NFF medical department

CAF and FIFA instructor Adegboye Onigbinde on Tuesday called for the establishment of a medical department in the Nigeria Football Federation (NFF).

Onigbinde made the call while delivering a paper titled “Areas of Fitness’’ at the ongoing NFF/CAF B-Licence Coaching Course, organised by the NFF technical department in Abuja.

According to Onigbinde, establishing a medical department will ensure that the medical staff are regularly trained for the day-to-day football challenge.

“This will ensure that they don’t just have a medical doctor for the football teams but a special football doctors,’’ he said.

Onigbinde, whose topic dwelt on intellectual fitness, psychology, medical, physiological, technical and tactical aspects of football, noted that a good coach was one who could improve the players’ intellectual fitness.

He also said that coaches should be able to think constructively to impact positively on players, noting that coaching went beyond what happened on the pitch.

The former Super Eagles coach added that coaches should have speed, strength, flexibility, agility, stamina and endurance to be able to succeed.

Amodu Shaibu, Technical Director, NFF said that coaches were to be blamed for the misfortunes of their teams, since they take the glory when the teams perform well.

Shaibu urged the coaches to be alive to their responsibilities in order to be reckoned with in their coaching career.

The News Agency of Nigeria (NAN) reports that Robinson Okosun, a former psychologist of the Eagles also presented a paper on the psychological aspects of the game.

James Peters, another instructor and former Assistant Technical Director, NFF presented a paper on “the qualities and responsibilities of the coach/manager’’.

No fewer than 60 participants are taking part in the coaching course which started from Nov. 16 to Nov. 28.

Understand the Spirit of Homeopathy Before Giving Medicine!

Most people that turn to homeopathy for their treatment do not know how this system of medicine works. Rather they have many misconceptions as regards the realm of action of homeopathy. Many people misinterpret it as a science that can be easily learnt at home and applied to the sick at home. Well, it is true that the person who understands the gist of homeopathy in a precise manner can apply it easily. But when I find people often taking lots of homeopathic medicines through the day, I find it strange. Often when we prescribe, we suggest people to take exactly the dosage suggested in the prescription of the given remedy. For no reason without consulting us, they should take another dose of the remedy. It is very strictly maintained, especially in online consultations where we don’t meet the patients on one to one basis.

Homeopathic medicines act best when taken minimally!

This point needs to be stressed by the homeopathic physicians. When some patients ask us why we give so minimum dose of the medicine, it is many times hard to convince them that it is the way homeopathy acts; not only acts but acts better! The potentized dose of any homeopathic medicine is strong enough to stir the vital force latent inside the individual. We should give this dose enough time to act and exhaust its action. Only then we should think of the repetition or changing the remedy as per the requirement.

Also, the remedies react to each other- positively or negatively. The “Relationship of the Remedies” is one of the most important topics of study in homeopathy. Without knowing which remedy follows well and which remedy antidotes or is inimical, we should not take another medicine during the primary course of a remedy. I had prescribed one of my patients Merc sol 200 for obvious reasons and symptom complex including mouth ulcers. He was a novice studying homeopathy on his own and as he developed slight sore throat the next day morning, he took a dose of Hepar sulph 200. The two remedies contradict each other and his sore throat increased while his mouth ulcers aggravated too. He was perplexed. That is when we told him the reason why this happened and having faith in the science, he listened to our suggestion and continued exact therapy suggested afterwards.

But this doesn’t happen always. Some of the times I have lost some of my patients that cannot be convinced of the minimal dose concept. They find it strange and knowing some small things about certain remedies, take the remedies on their own during the course suggested by us.

This type of attitude doesn’t do any good to anyone. Moreover, we form certain tedious symptom-complexes by this haphazard consumption of homeopathic medicines. And that is when the case is unnecessarily complicated. It is often stressed by stalwarts of homeopathy that a messed up case with homeopathic medicines is the hardest case to treat efficaciously. There is absolutely blurred picture after such recurrent doses and the patient cannot be cured of his malady for longer duration. I have particularly observed this in skin and respiratory problems.

To treat homeopathically, a homeopathic physician has to have lot of patience. Wait and Watch is many times the best policy instead of changing the drugs repeatedly and handling the case in childish manner. This does not mean during a chronic case, we cannot institute any acute remedy for acute complaints or acute exacerbation of chronic complaints. But the remedy selected has to be in sync with the earlier administered remedy and should be given after an appropriate gap. Especially after we administer nosodes, we must wait at least for two weeks before we give any other remedy. Nosodes are potent medicines and they need time to act. We must not interfere in their action. If no emergency comes, we should wait even longer before we resort to another remedy. Due to nosode’s intercurrent action, many times the earlier remedy starts acting much better and there remains no need of any new remedy at all.

All these points are considered when the homeopaths ask you to give gaps between your regimes. Always remember that classical homeopathy advocates no repetition of dose or remedy if the patient has started feeling better. We should wait till the action is over and only then think of the next possible course.

Do not fall to the myth that homeopathic medicines absolutely don’t have any side effects. Yes, it is completely untrue. Of course when given judiciously, the homeopathic medicines act the best without inflicting any ups and downs in the patient’s condition. However, haphazard consumption of homeopathic medicine if doesn’t lead to short term side effects, will certainly lead to some long term problems that can be hard to tackle later on! So, be careful and judicious while applying homeopathy in you or in other people. Comprehending the spirit of homeopathy is must before we act!
